First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Our goal is to ensure that you receive thorough information and recommendations, tailored to meet your specific needs. Our time investment in patient education enables us to render the best possible outcome for your dental treatment.

Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day. 

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, bisphosphonates etc.).

Before your initial appointment:

Please complete our New Patient Registration & Medical Questionnaire prior to your scheduled appointment. 

If you are unable to complete the form in advance, please arrive 30 minutes prior to your scheduled appointment time. 

Please provide the following information at the time of your appointment:

  • Referral slip
  • X-rays and/or CBCT scans if applicable
  • A list of medications you are presently taking
  • Insurance information
  • Government Issued Identification (Health card or drivers license) 


    IMPORTANT:
    All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

X-Rays

If your dentist or physician has taken x-rays you may request that they forward them to our office. In most cases x-rays can be emailed to our office at [email protected]. If additional x-rays are necessary they can be taken at our office.
